HealthChoice is a cost sharing dental insurance plan. Many HealthChoice members are public service employees such as firefighters, police officers, and teachers. HealthChoice dental plans are provided by the Employees Group Insurance Division (EGID), previously called the Oklahoma State & Education Employees Group Insurance Board (OSEEGIB).
Like most dental insurance plans, the plan pays after a deductible is met. HealthChoice members have a $25 individual deductible or a combined $75 family deductible per calendar year. For basic and major treatment, patients are responsible for a percentage of the cost for covered health care services. The calendar year maximum benefit amount per person is $2,500 for non-orthodontic procedures. Non-covered services and amounts above the $2500 maximum are the patient’s responsibility.

Revitalize Dental Implants is a Network Provider for HealthChoice. Utilizing the services of a Network Provider will result in a higher level of benefits which means less out-of-pocket cost for the patient.
Basic restorative procedures including basic extractions, wisdom teeth removal, and general anesthesia are covered at 85% of allowable fees with a Network Provider vs. 70% with an out of network provider.
Major restorative procedures including dental implant placement are covered at 60% of allowable fees with a Network Provider vs. 50% with an out of network provider. As a Network Provider, Dr. Templeton will file your dental claims for you.
